HOW TO REGISTER
WITH THE PRACTICE
Providing you live within our
catchment area and
our Practice is accepting new
patients you will be able to apply
to join the Practice.
Our
reception staff will supply forms to
complete or download the GMS1 and
New Patient Questionnaire forms (see
the link below) and post or drop
them into the surgery. You will then
be asked to attend for a new patient
check.
New Patient Screening
Within 28 days of registering with
the practice for the first time, you
will be invited to attend an initial
health screen with our Healthcare
Assistant. This is strongly
recommended.
